We are seeking a compassionate and experienced Physician (MD/DO) to provide high-quality primary care services to residents in assisted living communities throughout the Albany region of New York.

Position Options

Part-Time Opportunity

• 2–3 full days per week
• Travel between Hoosick Falls, NY and Cambridge, NY

Full-Time Opportunity

• 4–5 full days per week
• Travel between Hoosick Falls, NY, Cambridge, NY, and Troy, NY

What We Offer

Required Qualifications

• Active New York State Physician (MD/DO) license
• Active DEA registration
• Currently enrolled and credentialed with Medicare and Medicaid (required)
• Ability to practice independently
  • NPs must meet New York State requirements, including a minimum of 3,600 clinical practice hours
• Primary care experience required

Preferred Qualifications

• Geriatric care experience
• Assisted living, long-term care, or skilled nursing experience
