Job Responsibilities
• Diagnose and manage acute and chronic conditions
• Develop and oversee treatment plans
• Prescribe medications and monitor ongoing care
• Promote preventive care and patient education
• Collaborate across care teams
Qualifications
• Board Certified or Board Eligible (Family Medicine or related)
• Active or eligible Indiana medical license
• Active DEA with Indiana prescribing authority
