Primary Care-Adults only

We are seeking a compassionate, experienced Primary Care Physician (MD/DO) to join a collaborative team focused on providing comprehensive, holistic care to older adults in a PACE (Program of All-Inclusive Care for the Elderly) model.

In this role, you’ll be responsible for managing the medical care of participants at your assigned day center, with occasional visits to homes, hospitals, or nursing facilities as needed. You’ll work closely with an interdisciplinary team, including social workers and therapists, to make thoughtful, value-based care decisions for a geriatric population.

Key Responsibilities:

• Conduct comprehensive and episodic assessments

• Provide clinical oversight and collaborate with CRNPs and care teams

• Participate in daily team meetings to guide care plans

• Coordinate care across settings (home, hospital, nursing facilities)

• Be available for rotating on-call coverage

Ideal Candidate Has:

• Experience in palliative care, geriatrics, hospice, or chronic disease management

• A whole-person, value-based care approach

• Strong collaboration and communication skills

• Comfort making and explaining complex medical decisions

• MD or DO with active license (or ability to obtain one)
