| SITE INFORMATION: ◊ Pertinent Facility Information: COVID VACCINE REQUIRED ◊ Housing/Travel Accommodations provided ◊ Credentialing Timeframe: 60 days JOB INFORMATION: ◊ Total Number of FTE’s needed? 1.0 – need ~12 shifts per month ◊ EMR: EPIC – must be proficient ◊ Coverage Dates (Specific, long term only): July target start; need to be expected to last 6+ months ◊ Shift Schedule Details & Shift Duration: 9a-9p, 9p-9a; schedule is variable ◊ Type of Patients/Cases: all ages, primarily adult ◊ Patients per Day/Volume: 20; approx. 9k annual volume ◊ Responsible of Mid-Level Providers: no ◊ Back up Specialties to EM Department: all specialties available with Sacred Heart 1.5 hours away as needed ◊ Trauma Level: Level 4 ◊ Transfers (Helicopter, Ground, Where to? Springfield Eugene, OR ◊ Fast Track (Yes or No): No ◊ Physician Coverage (Single, Double, or Triple): Single ◊ Run Codes:Yes ◊ Scribes: No ◊ Miscellaneous Information: Florence, OR is a retirement community, and this facility has 21 total inpatient beds and 11 ED beds. Fairly low volume and acuity is expected, and the largest PeaceHealth facility is located approximately 1.5 hours away in Springfield when transfers are necessary. There is normally a provider on call for part of each shift in case it's too busy for a solo provider. REQUIREMENTS: ◊ What is the minimum length of commitment requirement? ◊ Certifications/Qualifications Required (ACLS, BLS, MSQA, TEE, etc.): ACLS, PALS, ATLS ◊ Education Requirements (particular Residencies, particular Fellowships, etc.): ER residency ◊ Board Certified Required: Yes Specific Specialty Board Required? ABEM or AOBEM If BC is not required, Number of Years of Board Eligibility Allowed since Residency/Fellowship Completion (0-3 years, 0-5 years, etc.): 3 years ◊ IMLC State: No ◊ Active State License Required?
