100% Advanced Heart Failure position with a dedicated focus on heart failure care
Dedicated team support, including:
- Heart Failure Coordinators
- Transplant/VAD Coordinators
- Outreach Coordinator
- Social Workers
- Inpatient and outpatient Advanced Practice Providers
Dedicated Transplant and VAD program with established clinical support teams
24/7 Critical Care coverage
Established Cardiogenic Shock program with opportunities for continued growth and program development
Formal multidisciplinary Pulmonary Hypertension program
