PATIENT INFORMATION

REQUIREMENTS
- Active FL license
- Active DEA
- Active ACLS or BLS
- Completed Lee Attestation Form
- Clean NPBD report (within the last 6 months)
DUTIES & RESPONSIBILITIES
- Provide cardiology coverage at three campuses.
- Perform rounding duties on patients from 8:00 AM to 5:00 PM on assigned dates.
- Provide call coverage as scheduled (e.g., December 4th 5:00 PM to December 7th 8:00 AM).
- Complete required orientation and submit necessary documentation
- Adhere to all facility policies and credentialing requirements.
