Preference for 1 Full-Time Locum Provider
May Consider 2 Providers Working Opposite Schedules
Alternate Schedule Options May Include 1 Week On / 1 Week Off or 2 Weeks On / 2 Weeks Off

Washington Medical License Preferred
Candidates With an Active IMLC Letter of Qualification May Be Considered
DEA Registration - Required
Washington CSR - Required
ACLS - Required
ACLS Must Be From AHA or American Red Cross as Applicable
Minimum of 2 Years of Experience - Required
Epic Experience - Required
COVID-19 Vaccination - Required, No Exemptions
Recent Clinical Activity Requirements
Must Provide Documentation of at Least 100 Cases From the Previous 24 Months
Case List Must Be Representative of the Scope and Complexity of the Privileges Requested
