- Active, unrestricted IL license OR IMLC
- Clean background/DEA/malpractice/NPDB
- Board Certified or truly Board Eligible
- BLS & ACLS Certifications
- Proof of Covid Vaccines
- 2 References (one must be a supervisor)

- Availability for start/schedule/length of time
- Must be able to perform TEEs
*Start Date: As soon as credentialed
*Shifts: M-F, 8a-4:30p; 1-2 weeks per month; with some weekend call - F, 4:30p-M, 8a
*End Date: provide availability and what length of stay you can commit to
*Round trip mileage and lodging provideed
*Malpractice Insurance Provided (Occurrence)
