Requirements:
- Physicians must be board certified in their specialty, or board eligible and will become board certified within 5 years of becoming board eligible - [REQUIRED] (no exceptions)
- Valid MI license - [REQUIRED] - at time of employment.
- 4th year advanced endoscopy training required.
- DEA - [REQUIRED]
- BLS Certification and maintain recertification - [REQUIRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- Board Certified
