Dates Needed: Jan 20, Feb 22, March 22, April 12, 26, May 24
Credentialing time Frame: 90-120 days
Certificates must be AHA issued: ACLS
Hours: 7a-3p, 3p-5p, 5p-7p (depending on site)
Rotation: 4th provider needs for Watchman cases
Call Schedule: 7p-7a, 5:30p-7a (depending on which site). Limited OR call/must be open to occasional back-up call
Call Response: 20 minutes
Any Supervision: Some supervision & performing own cases

Cases: Peds, General, Ortho, Uro, Ophthalmology, ENT, Plastics, Podiatry, Cardiac, Endo, Vascular, GYN, Pain Management
Spinals, Blocks and Epidurals Required: Yes
Case logs Required: Yes - proof of epidurals, spinals and blocks
Number of Providers: 3 MDs/10 CRNAs
Number of OR's: 4-5 depending on site
EMR System: EPIC
Additional Information: Cardiac/TEE experience needed but not required. Local preferred
