Qualifications
- ABMS/AOA board certified or board eligible in Physical Medicine & Rehabilitation (PM&R) or Anesthesiology.
- Fellowship in Pain Medicine
- Unrestricted licensed to practice medicine in New York or willing to license.
- Active and unrestricted DEA registration
