Requirements:
- Board Certified or Board Eligible (will become Board Certified within 2 years) in Anesthesiology - [REQUIRED]
- MI License (or willing to obtain) - [REQUIRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- DEA (or willing to obtain) - [REQUIRED]
- ACLS & BLS (or willing to obtain) - [REQUIRED]
- State interest in full-time or part-time employment (note in "Additional Submission Details" field) - [REQUIRED]
