Requirements

✔ Board Certified or Board Eligible in Anesthesiology (BC within 2 years)
✔ Michigan License (or willing to obtain)
✔ DEA (or willing to obtain)
✔ BLS & ACLS (or willing to obtain)
✔ NPDB Self-Query (within 30 days) required at presentation
✔ Specify Full-time or Part-time interest
