Located in Crown Point, IN; Chicago 45m
Overview
* The health center is seeking a Certified Nurse Midwife (CNM) for its Crown Point clinic
* Work within the Patient Centered Medical Home (PCMH) model alongside the patient care team and collaborating physicians
* Assess health status, form diagnoses, order appropriate diagnostics and implement treatment plans
* Must meet the health center's credentialing requirements and demonstrate competency as a CNM
Qualifications
* Certified Nurse Midwife, Bachelor's degree in Nursing minimum
* Indiana RN license, Indiana Nurse Midwife credential, Indiana APRN license and Indiana CSR license
* Two years of experience in a healthcare setting preferred
