Neurology APP – Memory Care - Findlay, OH NW Ohio ### The Opportunity Seeking a full-time Advanced Practice Provider to join the Neurology team in Findlay, Ohio, expanding access to neurology care while helping build a dedicated memory care program. ### Position Highlights
- Focus on memory care and cognitive disorders
- Average 12 patients per day, allowing for longer appointment times
- Help improve early detection, diagnostic accuracy, caregiver preparedness, and safe aging in place
- Coordinate care and connect patients and caregivers with community resources
- Shared office phone call, 1:3 rotation
- Hospitalist group manages inpatient admissions ### Qualifications
- Certified Nurse Practitioner or Physician Assistant
- Minimum 2 years of neurology or memory care experience preferred
- Candidates without general neurology experience may be considered, with an expectation to develop proficiency in general neurology ### Compensation & Benefits
- Base salary: $120,000 – $145,000 based on experience
- $3,500 annual CME/Board Certification allowance
- 200 hours vacation/CME time

- 403(b) retirement plan with 4% employer match beginning Year 2
- Sick time accrual up to 600 hours
