Schedule:
- Plover clinic
- Monday, Wednesday, Thursday, and Friday, 8:00a-4:30p
- Monday, Wednesday, Thursday, and Friday, 8:00a-4:30p
- Portage clinic
- Tuesday, 8:00a-4:30p
- Tuesday, 8:00a-4:30p
Patient Population: Adult
EMR: EPIC
Credentialing: 90 days
Requirements:
- Active Wisconsin licensure
- Board certification
- AHA BLS certification
- Department of Transportation (DOT) certification with FMCSA digital upload experience
- Minimum 1-year Occupational Medicine experience or equivalent
