🩺 Key Responsibilities

• Evaluate and manage patients with acute and chronic kidney disease

• Conduct comprehensive history & physical examinations

• Manage CKD, ESRD, hypertension, electrolyte disorders, and renal complications

• Dialysis patient management (if applicable)

• Order and interpret laboratory tests and diagnostic studies

• Adjust medications based on renal function and treatment response

• Provide patient education on disease management and lifestyle modification

• Coordinate care with primary care providers and specialists

• Maintain accurate and timely EMR documentation

📋 Qualifications

✅ Licensed Nurse Practitioner or Physician Assistant in New York
✅ Board Certified (AANP/ANCC or NCCPA)
✅ Active DEA license (if required)
✅ Prior nephrology, internal medicine, or hospital medicine experience preferred
✅ Strong clinical decision-making skills
✅ Excellent communication and team collaboration abilities
