Healthcare United is seeking an experienced Physician Assistant or Nurse Practitioner for an outpatient primary care float opportunity based in Shelton, Connecticut.
This role requires at least five years of provider experience and is ideal for an APP with strong primary care, internal medicine, family medicine, urgent care, or outpatient medicine experience.
The incoming provider will support outpatient primary care needs, evaluate patients, manage acute and chronic conditions, provide preventive care, and work collaboratively with physicians, APPs, nurses, and clinical staff.
Job Responsibilities
• Float as needed to support clinic coverage and patient access
• Evaluate and manage adult or family medicine patients
• Conduct wellness visits, sick visits, chronic disease management, and preventive care
• Develop treatment plans and coordinate follow-up care
• Collaborate with physicians, APPs, nurses, and clinical staff
• Maintain accurate and timely documentation in the EMR
• Support continuity of care and patient satisfaction
Qualifications
• Active or eligible Connecticut APRN or PA license
• Minimum of five years of provider experience required
• Primary care, internal medicine, family medicine, urgent care, or outpatient experience preferred
• Strong independent clinical judgment
• Excellent communication and documentation skills
