An FQHC in New York City is looking for a licensed Nurse Practitioner (NP) and/or Physician Assistant (PA) to join their growing team in the Bronx.

About the Opportunity:

• Start Date: ASAP
• Assignment Length: 6 months w/ possibility of extension
• Schedule: Monday through Friday
• Specialty: Primary Care
• Setting: Outpatient Community Health Center

***Local providers only, familiarity with the Bronx is helpful.***

Responsibilities:

The Nurse Practitioner / Physician Assistant will:

• Identify and categorize each patient's age-specific grouping of needs, such as those infants, adolescent, or geriatric patients
• Take histories, perform physical examinations, give immunizations (as necessary) and does patient education and counseling
• Order appropriate lab work, draws blood for lab work if needed, and performs some lab tests such as wet prep microscopic exams
• Prescribe treatment for minor illnesses, and consults with physician regarding medication, chronic illness and referrals when appropriate
• Maintain legible and appropriate documentation and reviews records periodically
• Attend continuing education programs to increase skills and knowledge
• Attend provider meetings and participates in quality improvement programs and team assignments
• Perform other duties, as needed

Qualifications:

• NYS Nurse Practitioner and/or Physician Assistant (PA) license
• Board-Certified
