Mid- Level practitioner needed for a Care Navigator position in San Jose, CA
A dynamic University Health system in Northern CA is recruiting for a Practitioner with experience in Internal Medicine or Geriatrics and has a passion for working with a small population of complex senior patients. They will be the primary point of contact for high- risk, high-acuity Medical Managed Care patients included in the Care Navigator Program. They will provide care in various different settings including home visits, skilled nursing facilities hospital visits, and step- down acute care facilities. This position requires travel throughout the South bay area. The care navigator is expected to learn use and apply principles relating to Lean/Six Sigma. Population management, quality principles and practices, motivational interviewing and care pathways in their daily work. Should the Mid- Level practitioner have experience with EPIC EMR that is a plus. This is an employed position with the health system offering a competitive salary and benefits package.
