HIV Care Clinic Advanced Practice Provider Opportunity - Southern Tier, New York Seeking a full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join the HIV Care Clinic in Elmira, NY — delivering compassionate, patient-centered HIV care across the Southern Tier of New York and Northern Pennsylvania. Position Details

  • HIV care including diagnosis, treatment planning, medication management, and follow-up care
  • Management of patients with complex needs related to HIV and comorbid conditions
  • PrEP, PEP, and U=U counseling and preventive interventions
  • Collaboration with a multidisciplinary team of physicians, nursing staff, case managers, and social workers
  • Accurate clinical documentation in electronic health records
  • Participation in quality improvement activities and community outreach
