Job description Seeking a Nurse Practitioner or Physician Assistant for a unique, dual-role position combining hospital-based scribe responsibilities with direct patient care in a clinical setting in Sparta, IL. This role supports a hospitalist physician while also providing comprehensive care across a diverse patient population.


Position Overview/Key Responsibilities:

  • Scribe for hospitalist physicians, documenting patient encounters, orders, referrals, and care plans in real time
  • Provide direct patient care for all ages across hospital, clinic, long-term care, and home settings
  • Conduct exams, assessments, diagnostic evaluations, and develop treatment plans
  • Manage preventive, routine, and transitional care, including discharge planning and follow-ups
  • Educate patients on wellness and health management
  • Participate in telehealth visits, specialty procedures, and long-term care visits as needed
  • Flexible schedule with evenings, weekends, holidays, and on-call duties (60-minute response)
  • Adhere to HIPAA standards and maintain patient confidentiality


Qualifications:

  • NP or PA with appropriate state licensure and national certification
  • Master’s degree required (NP candidates)
  • Minimum 3 years of experience preferred but not required
  • Strong clinical, documentation, and communication skills
  • Knowledge of EHR systems and medical coding principles preferred
  • Current CPR, ACLS, and PALS certifications

Compensation/Benefits:
  • Salary Range: $108K to $158K Annually
  • $20K Signing bonus
