Job description

Job description Job Description:
Client is a small, independent, and growing medical group providing care to skilled nursing and long-term care facilities throughout the Puget Sound region. The organization is built around delivering thoughtful, relationship-based medical care that allows providers to develop meaningful, ongoing relationships with their patients.

Client emphasizes clinical judgment, autonomy, continuity of care, and consistent follow-through. Providers take time to understand each patient's health history, baseline condition, personal goals, family and care needs while building strong working relationships with nursing teams and facility staff.

As the organization continues to grow, Client is seeking an Advanced Registered Nurse Practitioner (ARNP) or Physician Assistant-Certified (PA-C) who is passionate about providing high-quality care to adult and geriatric patients in post-acute and long-term care settings.

The ARNP or PA-C will provide comprehensive primary and post-acute medical care to adults residing in skilled nursing facilities and long-term care communities throughout the Puget Sound region.

Most providers spend their workday at one assigned facility, allowing them to establish continuity with patients and develop strong relationships with nursing staff, facility leadership, families, and other healthcare professionals. Physicians rotate through facilities and remain available for collaboration and clinical support.

Responsibilities include managing acute and chronic medical conditions, conducting admissions and follow-up visits, completing required regulatory visits, coordinating discharge care, managing medications, and collaborating with interdisciplinary care teams.

This role is well suited to a provider who values clinical independence, meaningful patient interactions, and the opportunity to contribute to a growing medical practice.

Job Responsibilities

Key Responsibilities:
Patient Care and Clinical Management:
Provide comprehensive medical care to adult patients in skilled nursing and long-term care settings.
Evaluate and manage acute medical concerns and chronic health conditions.
Conduct patient admissions, follow-up visits, regulatory visits, and discharge visits as appropriate.
Assess medically complex patients, including individuals with multiple chronic conditions and ongoing care needs.
Review laboratory results, imaging studies, medication regimens, and clinical documentation.
Develop, implement, and update individualized treatment plans based on patient needs and clinical findings.
Perform procedures when clinically appropriate and within the provider's scope of practice, training, privileges, and facility capabilities. Additional training may be provided when needed.

Care Coordination and Collaboration:
Communicate effectively with nursing staff, patients' families, specialists, pharmacies, and other members of the healthcare team.
Coordinate care with facility leadership, attending physicians, and other treating providers.
Participate in interdisciplinary discussions to support continuity, safety, and quality of care.
Build collaborative relationships with facility staff to improve communication and address patient needs.
Work with rotating physicians and clinical colleagues to obtain guidance and support when needed.

Documentation and Clinical Administration:
Maintain accurate, thorough, and timely clinical documentation.
Complete documentation within 48 hours, with a goal of same-day completion whenever possible.
Use available scribe services and advanced AI documentation tools to support efficient clinical workflows.
Review patient records and clinical information to support appropriate treatment decisions and continuity of care.
Maintain compliance with applicable clinical, organizational, facility, and payer documentation requirements.

On-Call Responsibilities:
Participate in a shared, phone-only on-call rotation.
Respond to clinical questions and concerns in accordance with organizational protocols and patient care needs.
Collaborate with physicians and other healthcare professionals when additional clinical support is required.

Qualifications

Required Qualifications:
Current Washington State licensure as an ARNP or PA-C, or eligibility to obtain the required license.
Current national board certification.
Prescriptive authority, as applicable to the provider's role.
DEA registration or eligibility to obtain registration when required.
Ability to meet payer, facility, and organizational credentialing requirements.
Relevant experience caring for adult or geriatric patients.
Ability to assess and manage medically complex patients with multiple chronic conditions.
Strong independent clinical judgment and sound decision-making skills.
Excellent communication and collaboration skills.
Strong organizational skills and attention to clinical documentation.
Ability to work independently while maintaining effective communication with physicians and interdisciplinary care teams.

Preferred Qualifications:
Experience in skilled nursing facilities or long-term care environments.
Background in geriatrics, primary care, internal medicine, hospital medicine, or a related clinical setting.
Experience managing chronic conditions, acute changes in patient status, medication regimens, and post-acute care needs.
Familiarity with regulatory visits, admissions, discharge planning, and interdisciplinary care coordination.

Working Hours

Schedule and Travel Requirements:
Provide in-person care at skilled nursing and long-term care facilities throughout the Puget Sound region.
Spend most workdays at one assigned facility to support continuity of care and strong relationships with facility teams.
Travel between facilities when required by patient needs, coverage requirements, or operational priorities.
Adapt to changing assignments and patient care demands as necessary.
