Job Responsibilities
• Evaluate and manage chronic medical conditions
• Address acute health concerns and develop treatment plans
• Collaborate with facility staff, families, and healthcare partners
• Provide compassionate communication and patient education
• Maintain accurate clinical documentation
• Deliver safe, personalized care focused on improving quality of life
Qualifications
• Current, unrestricted Nurse Practitioner or Physician Assistant license
• Georgia NP or PA license (or eligibility)
• Master's degree from an accredited NP or PA program
• Minimum 3 years of clinical experience
• Ability to provide onsite patient care in Decatur, GA
Preferred:
• Experience with senior populations
• Geriatric medicine experience
• Hospice or home health background
• Chronic disease management experience
