Seeking a NP/PA to provide comprehensive geriatric-focused care for residents in skilled nursing facilities.
Job Responsibilities
• Manage acute and chronic medical conditions
• Order, interpret, and document diagnostic studies
• Prescribe medications and therapies
• Apply evidence-based clinical guidelines
• Coordinate specialty referrals
• Educate residents and families on care plans and health maintenance
• Collaborate with multidisciplinary SNF teams
• Work with peer providers and supervising physicians
Qualifications
• Geriatric or SNF experience preferred
• Strong clinical and documentation skills
• Ability to work independently
