Seeking a NP/PA to provide comprehensive, geriatric-focused care for residents in skilled nursing facilities with daily onsite clinical coverage.
Job Responsibilities
• Manage acute and chronic medical conditions
• Order, interpret, and document diagnostic studies
• Prescribe medications and therapies
• Apply evidence-based treatment guidelines
• Coordinate referrals and specialty care
• Educate residents and families on care plans
• Collaborate with multidisciplinary SNF care teams
• Work closely with supervising physicians
Qualifications
• Experience in geriatrics, SNF, or long-term care preferred
• Strong clinical assessment and documentation skills
• Ability to work independently and collaboratively
