Physician Assistant – Chronic Care, Hybrid / In-Home Care
Richmond, VA
This Is Not a Clinic Job. And That’s the Point.
If you became an APP to practice autonomously, build real patient relationships, and actually improve outcomes—this role was designed for you.
We’re looking for APPs who wants to step away from rushed visits, crowded schedules, and productivity pressure, and instead deliver thoughtful, evidence-based care to complex patients where it matters most: in their homes.

$120,000 – $140,000 base salary (depending on experience) + generous Sign On bonus!

Job Responsibilities
Deliver in-home and virtual care for patients with CKD through ESRD
Perform comprehensive assessments, wellness visits, and focused follow-ups
Coordinate care with PCPs, specialists, and an integrated RN + Social Work team
Educate patients and families, close gaps in care, and prevent avoidable hospitalizations
Prescribe, order diagnostics, document efficiently, and practice at the top of your license
Travel statewide during the workweek (up to ~75% travel)
Qualifications
Active, unrestricted APP License
Board certified (AANP or ANCC)
2+ years of direct patient care experience
Comfortable working independently in the field
Willingness to travel and meet patients where they are
Strong clinical judgment, communication skills, and heart for complex care
