A healthcare company in Brookyln is actively seeking a licensed Nurse Practitioner (NP) to work between two of their nursing sites in Brooklyn.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: Flexible
• Setting: Nursing Home / Managed Long Term Care Facility
• Patient Population: Geriatric
• Patient Volume: Small caseload at each nursing home

Responsibilities:

The Nurse Practitioner (NP) will:

• Manage members with acute change in condition
• Review and manage pharmacotherapy of each member
• Prescribe appropriate medications, including controlled substances
• Identify, treat, and document changes in condition
• Order, review, and/or follow-up with treatments and diagnostic testing
• Complete timely health risk assessments
• Complete routine monthly comprehensive assessments
• Perform other duties, as needed

Qualifications:

• 1+ year of recent Clinical experience with the Elderly population
• Master's Degree
• NYS Nurse Practitioner (NP) license
• DEA registration
• Previous experience in I-SNP, HMO
• Working knowledge of Clinical Documentation
• Solid assessment, clinical, and documentation skills
• Great interpersonal skills
• Excellent communication skills (written and verbal)
• Strong attention to detail
• Highly organized
