- Compensation: Competitive Salary- $ 110,000- $ 130,000 annually, based on experience Essential Duties and Responsibilities: -Provide routine primary care, treatment, family planning, and prenatal services as requested. -Order, interpret, and evaluate diagnostic tests to assess patient needs. -Refer patients to Physicians or specialized resources as appropriate. -Participate in quality improvement initiatives and complete required training. -Perform other related duties as assigned. *Bilingual preferred

- Additional Info: Join us as a core member of the Collaborative Care Management team, working alongside the patient's medical provider, a consulting Psychiatric Consultant, and the broader primary care team. You will be responsible for supporting and coordinating the mental and physical health care of patients on an assigned caseload, in partnership with the patient's medical provider. Required Experience: Qualifications: -Current APRN licensure in Illinois with FNP certification -Current BLS certification -Valid driver's license and auto insurance, ability to travel to all Aunt Martha's locations -Availability for occasional evening or weekend emergencies
