An established healthcare system in Lyons, New York is seeking experienced Family Nurse Practitioners to join its brand-new Primary Care practice. Clinical Responsibilities
Provide comprehensive primary care services to patients and families
Perform routine physical examinations and wellness visits
Diagnose and manage acute and chronic medical conditions
Conduct preventive health screenings and assessments
Develop individualized treatment and disease management plans
Order and interpret diagnostic testing
Prescribe medications and monitor treatment effectiveness
Provide patient education on preventive care and healthy lifestyles
Coordinate specialty referrals and follow-up care
Collaborate with physicians and multidisciplinary clinical teams
Establish long-term relationships with patients
Maintain accurate and timely clinical documentation

Qualifications
Certified Family Nurse Practitioner (FNP)
Current New York State Nurse Practitioner license
Board Certification as a Family Nurse Practitioner
Prior Primary Care experience preferred
