- See patients independently, with physician collaboration as needed
- Build long-term relationships with patients through longitudinal care
- Focus on in-clinic primary care, care coordination and population health; specialists and hospitals handle specialty and acute care
- Care team: physician, MAs, RNs and care managers
- Outpatient only
- NCCPA certified
- 2 years of primary care experience
- Active, non-probationary PA license
- Active DEA
- Medical, dental and vision
- PTO, retirement plan and wellness programs

About the Opportunity
- Full-time, permanent outpatient primary care PA role, 40 hours/week- Value-based care model keeping older adults on Medicare healthy and out of the hospital

Requirements
- Graduate of an accredited Physician Assistant program- NCCPA certified
- 2 years of primary care experience
- Active, non-probationary PA license
- Active DEA

- Knowledge of HCC documentation, ICD-10 coding and Health Risk Assessments (HRAs)Compensation & Benefits
