Ammon, Idaho, Physician Assistant Primary Care Job

• Schedule: Monday–Friday, 8:00 AM–5:00 PM

• Employment Type: Full-time, 5 days per week

• Patient Visits: In-home, in-clinic, and telehealth

• Travel Requirements: Home visits within a 30-mile radius of the provider's home region

• Flexibility: Must be available to provide coverage in other regions as needed

• State License: Active Idaho license required for Physician Assistants

• Board Certification: Required

• DEA / Prescriptive Authority: Required after hire

• Certifications: Active CPR/BLS required

• Driver's License: Current Idaho driver's license required

• Bilingual: English/Spanish preferred

• Minimum Experience: 2+ years of clinical experience

• Accepted Specialties: Primary Care, Family Medicine, Internal Medicine, Gerontology, or Palliative Care

• Preferred Experience: Home-based care

• Knowledge of chronic care management and medication options

• Conduct annual wellness visits and manage high-risk senior patients, primarily in their homes.

• Provide in-clinic and telehealth visits as needed.

• Document medical histories, symptoms, medications, and patient encounters.

• Perform screenings and assessments.

• Collaborate with the primary care team to coordinate comprehensive patient care.

• Educate patients on preventive care and treatment plans.

• Maintain timely records and comply with HIPAA requirements.
