Nurse Practitioner Opportunity in Sandusky, OH
Position Overview:
Seeking a Nurse Practitioner to join a multidisciplinary healthcare team providing evidence-based care for acute and complex chronic conditions. This full-time position primarily focuses on outpatient care with potential involvement in inpatient coordination, offering an opportunity to practice in a collaborative, patient-centered environment.
Practice Details:
Full-time employed position
Primarily outpatient practice
Collaborate with physicians and multidisciplinary care teams
Coordinate inpatient care and discharge planning as needed
Evidence-based clinical practice
Utilize established clinical pathways and protocols
Clinical Responsibilities:
Perform comprehensive patient evaluations and physical examinations
Diagnose and manage acute and chronic medical conditions
Order, interpret, and manage diagnostic testing
Develop individualized treatment plans
Prescribe medications and therapeutic interventions
Perform office-based procedures
Manage complex wound care and minor procedures
Coordinate care across the continuum
Maintain timely and accurate EMR documentation
Participate in quality improvement initiatives
Requirements:
Master's degree from an accredited Nurse Practitioner program
Active Nurse Practitioner license
DEA or eligibility to obtain
Eligible for credentialing
BLS certification
ACLS certification if required by assigned practice
Minimum of two years of experience preferred
Strong clinical decision-making and communication skills
