Port Jervis / Middletown, NY – Full-Time Provider
- NP
- Monday through Friday
- Provider would work between our Port Jervis and Middletown locations
- Full-time schedule with consistent assigned days/locations
Dutchess County, NY – Full-Time Provider
- NP
- Monday through Friday
- Coverage within the Dutchess County area, which may include Poughkeepsie, Wappingers Falls, Beacon, and surrounding locations
- Provider must be comfortable traveling between assigned facilities (although typically never in the same day)
Bronx, NY – Part-Time Provider
- NP
- Preferably two to three shifts per week
- Shifts are generally a minimum of four hours
- There is flexibility regarding which days are worked; however, once a schedule is established, we do expect consistency
