EXPERIENCED PRIMARY CARE NP/PA | MULTIPLE CONNECTICUT LOCATIONS

Experienced Nurse Practitioners and Physician Assistants are being recruited for permanent outpatient Primary Care opportunities across Connecticut.

• Bridgeport

• Additional Connecticut communities
Location availability and schedules vary by practice.
PRACTICE SNAPSHOT
• Traditional outpatient Primary Care
• Continuity-panel ownership
• Weekday-focused schedules
• Established clinical and administrative support
• Integrated specialty access
• Modern documentation and care-management resources
WHAT YOU’LL DO
• Build and manage a continuity patient panel
• Evaluate and treat acute conditions and chronic diseases
• Provide preventive care, screenings, immunizations, and counseling
• Order and interpret laboratory and diagnostic studies
• Manage medications, referrals, and longitudinal care
• Maintain timely, accurate clinical documentation
• Sustain an average volume of at least 18 patients per day
WHAT YOU’LL BRING
• Nationally certified Nurse Practitioner or Physician Assistant
• Connecticut licensure or immediate eligibility
• At least one year of traditional, panel-based Primary Care experience
• Documented responsibility for an ongoing patient panel
• Demonstrated ability to see at least 18 patients per day
Urgent care, telehealth-only, specialty-clinic, and inpatient experience do not substitute for the required traditional Primary Care background.
