Job description
We are seeking experienced Nurse Practitioners to provide comprehensive primary care services through in-home/house calls across the Bronx, Queens, and Brooklyn. This is a full-time, permanent opportunity focused on delivering quality care to patients in the comfort of their homes.
Job Responsibilities
Diagnose and manage acute and chronic medical conditions.
Develop and implement individualized treatment plans.
Perform physical examinations and order diagnostic testing as appropriate.
Prescribe medications and provide preventive care.
Provide patient and family education.
Coordinate referrals and follow-up care.
Maintain accurate and timely EMR documentation.
Collaborate with physicians and other healthcare professionals.
Qualifications
Current New York NP license.
National board certification.
Current DEA registration.
BLS certification.
Primary care experience preferred.
Willingness to travel to patients’ homes throughout the assigned service area.
