🔹 Daily Responsibilities
As a PA, you’ll provide comprehensive primary care for senior patients, including annual wellness visits, acute and chronic condition management, physical exams, medication management, screenings, and care coordination. You’ll develop treatment plans, provide patient education, coordinate referrals and follow-up care, complete HCC coding, maintain accurate EHR documentation, and collaborate with the interdisciplinary care team.
🔹 Requirements
- Active Montana Physician Assistant/Associate License
- Board-Certified PA
- 2+ years of clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care
- Acute/high-risk patient experience is a plus
- Home health experience is a plus
- DEA/Prescriptive Authority
- CPR/BLS Certification
