- 🕐 Full-Time | Monday–Friday | 8 AM–5 PM
- 👩⚕️ 8–10 clinic patients/day | 5–6 home visits/day

🔹 Daily Responsibilities
As a PA, you’ll evaluate and manage senior and high-risk patients, perform annual wellness exams, review medical histories and medications, conduct screenings, complete HCC coding, and develop individualized care plans. You’ll provide patient education, assist with hospital discharge transitions, coordinate follow-up care, collaborate with social workers and other healthcare professionals, and provide care in both clinic and home settings.
🔹 Requirements
- Current Oregon Physician Assistant/Associate License
- Board-Certified PA
- 2+ years clinical experience in Primary Care, Family Medicine, Internal Medicine, Gerontology or Palliative Care
- High-risk/acute care experience is a plus
- Home health experience is a plus
- DEA/Prescriptive Authority
- CPR/BLS certification
- Oregon Driver’s License
