We're looking for an experienced Medical Provider to join our growing team in Goodyear, Arizona. This is an opportunity to develop meaningful, long-term relationships with patients while practicing in a collaborative and progressive wellness environment.
The Medical Provider will evaluate patients, develop individualized treatment plans, and provide ongoing care focused on wellness, prevention, and achieving meaningful health outcomes.
This position is well suited for a provider with a strong background in primary care, hormone replacement therapy, weight management, and wellness or functional medicine who enjoys working closely with patients over the long term.

Job Responsibilities
Evaluate patients and obtain comprehensive medical histories.
Perform physical assessments and develop individualized treatment plans.
Provide ongoing patient management and follow-up.
Practice within appropriate clinical, regulatory, and professional standards.
Evaluate and manage patients seeking hormone optimization and wellness services.
Provide appropriate weight-management care and counseling.
Support patients with lifestyle, nutrition, and preventive health strategies.
Evaluate and manage patients as appropriate for wellness and functional medicine services.
Educate patients about treatment options, expected outcomes, risks, and ongoing care.
Monitor patient progress and adjust treatment plans based on clinical findings and patient response.
Maintain accurate and timely medical records.
Build strong, trusting relationships with patients.
Collaborate with the clinical team to provide a consistent, high-quality patient experience.
Stay current with relevant medical knowledge, treatment approaches, and applicable standards of practice.
What We're Looking For:
We're seeking a compassionate, confident, and experienced provider who is passionate about helping patients achieve their health and wellness goals.
Qualifications
5+ years of primary care experience
2+ years of hormone replacement therapy experience
2+ years of weight-management experience
2+ years of functional medicine or wellness medicine experience
Experience developing long-term relationships with patients.
Strong clinical judgment and communication skills.
Patient-centered approach to care.
Ability to work independently while collaborating effectively with a multidisciplinary team.
Provider Credentials:
We welcome qualified Advanced Practice Providers (APPs), MDs, and DOs whose education, licensure, training, and experience meet applicable Arizona requirements for the services provided.
Candidates must maintain all required professional licenses, certifications, and credentials necessary to practice within their scope.
Schedule & Compensation:
Part-time or full-time contract position
24–36 hours per week
Flexible schedule, typically 3–4 days per week
APP: Salary/hour
MD/DO: Salary/hour
