Job description The Physician Assistant (PA) delivers high-quality, patient-centered primary care under the supervision of the Medical Director – Clinical Services. This role plays a key part in ensuring excellent clinical outcomes by coordinating care, maintaining regulatory compliance, and collaborating with interdisciplinary teams to support organizational goals.

Job Responsibilities

Clinical Care:
Provide comprehensive primary care services to assigned and referred patients.
Collaborate with case managers and healthcare professionals to coordinate patient care.
Deliver cross-coverage support across programs, including care in clinics, patient homes, and skilled nursing facilities (SNFs).
Participate in on-call rotations (including holidays), offering both telephonic guidance and in-person evaluations as needed.
Lead and contribute to family discussions, including conversations around end-of-life care and hospice services.

Documentation & Compliance:
Maintain accurate, timely, and complete documentation within the Electronic Health Record (EHR) system (NextGen).
Adhere to all applicable healthcare regulations, including HIPAA and Fraud, Waste, and Abuse (FWA) standards.
Communicate patient information effectively using approved channels (EHR, phone, email, and messaging platforms).
Uphold strict patient confidentiality and data security practices.

Collaboration & Organizational Support:
Contribute to organizational initiatives such as HCC and 5-Star quality programs.
Participate in Quality Improvement and Utilization Review activities.
Communicate professionally with leadership, providers, staff, patients, and families.
Attend team meetings and support departmental and organizational objectives.
Follow established procedures for scheduling and time-off requests.
Perform additional duties as assigned.

Qualifications

Current, unrestricted California licensure as either:
A Nurse Practitioner (RN with advanced degree and certification), or
A Physician Assistant licensed by the Medical Board of California from an accredited program.
Active DEA registration.
Proven experience in primary care.
Strong understanding of managed care systems.
Ability to work in a full-time or hybrid telecommuting environment, based on organizational needs.

Physical Requirements:
Prolonged sitting (approximately 70% of the workday).
Standing and walking for extended periods (15%+ each).
Ability to lift up to 20 pounds.
Frequent bending, occasional kneeling.
Adequate hearing and vision for effective communication and computer use.
Ability to work in a technology-driven environment, including extended computer use (6+ hours daily).
Reaching and hand dexterity required for routine tasks.
