A Central New York-based healthcare provider is seeking a licensed Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in several counties.
About the Opportunity:
- Schedule: Weekdays and weekends
- Hours: Minimum 9 hours/week
- Setting: Community-based
Responsibilities:
- Reviewing and recommending on consumer assessments
- Maintaining knowledge of regulatory changes and clinical quality
- Validating assessment outcomes using clinical skills
- Collaborating on quality assurance and improvement initiatives
- Supporting program goals through education and communication
Qualifications:
- 2+ years of experience as an APP
- Master's Degree (NP or PA Program)
- Active New York State License
- Active Medicaid Number
- BLS Certification
- Computer literacy and tech navigation
- Experience with telehealth assessments
- Strong written and oral communication
- Interpersonal skills and adaptability
Desired Skills:
- Experience with Adult/Geriatric populations
- Knowledge of Long-Term Care programs
