Job description
We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in several counties. This position supports the Independent Practitioner Panel (IPP) and involves a combination of remote and in-field duties.

- Hours: Minimum 9 hours/week

Responsibilities:
The Nurse Practitioner (NP) or Physician Assistant (PA) will be responsible for:
- Reviewing and recommending on consumer assessments
- Maintaining knowledge of regulatory changes and clinical quality
- Validating assessment outcomes using clinical skills
- Collaborating on quality assurance and improvement initiatives
- Supporting program goals through education and communication
Qualifications:
- 2+ years of experience as an APP
- Master's Degree (NP or PA Program)
- Active New York State License
- Active Medicaid Number
- BLS Certification
- Computer literacy and tech navigation
- Experience with telehealth assessments
- Strong written and oral communication
- Interpersonal skills and adaptability
- Knowledge of long-term care programs (preferred)
Desired Skills:
- Experience with adult/geriatric populations
- Master's Degree (NP or PA Program)
- BLS Certification
- Knowledge of long-term care programs
