We are seeking a Nurse Practitioner (NP) or Physician Assistant (PA) for a role focused on providing community-based long-term care services in several counties. This position supports the Independent Practitioner Panel (IPP) and involves a combination of remote and in-field duties.

About the Opportunity:

  • Hybrid or Remote: Hybrid
  • Shift: Variable
  • Schedule: Weekdays and weekends
  • Hours: Minimum 9 hours/week
  • Setting: Community-based

Responsibilities:

The Nurse Practitioner (NP) or Physician Assistant (PA) will be responsible for:

  • Reviewing and recommending on consumer assessments
  • Maintaining knowledge of regulatory changes and clinical quality
  • Validating assessment outcomes using clinical skills
  • Collaborating on quality assurance and improvement initiatives
  • Supporting program goals through education and communication

Qualifications:

  • 2+ years of experience as an APP
  • Master's Degree (NP or PA Program)
  • Active New York State License
  • Active Medicaid Number
  • BLS Certification
  • Computer literacy and tech navigation
  • Experience with telehealth assessments
  • Strong written and oral communication
  • Interpersonal skills and adaptability
  • Knowledge of long-term care programs (preferred)

Desired Skills:

  • Experience with adult/geriatric populations
  • Master's Degree (NP or PA Program)
  • BLS Certification
  • Knowledge of long-term care programs
