Lexington Health is seeking Family Medicine Advanced Practice Providers (PA or NP) to join our expansive primary care network across the Midlands of South Carolina. Our APPs practice in a supportive, team-based environment with strong physician collaboration, no inpatient responsibilities, and predictable schedules.
With over 1.3 million annual patient visits across more than 70 practices, Lexington Health provides the scale, stability, and resources needed to build a meaningful and sustainable outpatient practice.

Practice Highlights
- Outpatient-only primary care
- Team-based model with physician and APP collaboration
- Limited call (phone only)
- Onsite lab, imaging, and specialty referral support
- EPIC EHR with dedicated onsite support
- Preventive care and wellness visits
- Chronic disease management
- Acute illness evaluation and treatment
- Minor procedures and in-office diagnostics
- Certified PA or NP
- Eligible for South Carolina licensure
- Primary care experience preferred; new graduates considered
- Day ONE medical, dental and life insurance benefits
- $2,000 annual CME Allowance
- Health care and dependent care flexible spending accounts (FSAs)
- Employees are eligible for enrollment into the 403(b) match plan day one. LHI matches dollar for dollar up to 6%.
