Salary Range: $115k–$150k (depending on experience)

A growing community-based healthcare organization is seeking an NP or PA to join its primary care team. This role supports a diverse patient population and delivers comprehensive outpatient care within a mission-driven, federally qualified health–style environment. Multiple openings are available across Brooklyn and Staten Island.

Position Overview

The clinician will provide ongoing primary care services, build long-term patient relationships, and support culturally responsive care for individuals and families. Fluency in Cantonese is strongly preferred to meet the needs of the surrounding communities.

This is a full-time, weekday position with a stable schedule. No nights or major weekend commitments!

Key Responsibilities

  • Deliver routine primary care for patients of varying ages, including health assessments, preventive visits, chronic disease management, and follow-up care.
  • Provide culturally aligned patient education and support, particularly for Cantonese-speaking individuals.
  • Review diagnostic results, coordinate referrals, and collaborate with the clinical team to ensure continuity of care.
  • Maintain accurate and timely documentation within the electronic health record.
  • Participate in quality improvement and population health initiatives that support community wellness.
  • Work closely with nursing staff, care managers, and other providers to optimize patient outcomes.
  • Support walk-in and same-day care needs as appropriate.

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in New York.
  • Experience in primary care, family medicine, or community health strongly preferred; new graduates considered.
  • Cantonese language proficiency highly preferred to serve local patient populations.
  • Strong clinical judgment, communication skills, and a commitment to community-based care.
  • Comfort working in a fast-paced outpatient setting.
