Key Responsibilities:

• Provide high-quality outpatient clinical care to patients of all ages.

• Conduct evaluations, diagnose and manage acute and chronic conditions, and develop individualized treatment plans.

• Collaborate closely with physicians, nurses, and support staff to ensure coordinated patient care.

• Maintain accurate and timely documentation in the electronic medical record.

• Participate in quality improvement initiatives and support the organization’s mission to deliver exceptional patient-centered care.

Qualifications:

• Licensed Nurse Practitioner or Physician Assistant in Oregon (or eligibility for licensure).

• Previous clinical experience preferred; candidates with two years of experience may qualify for moving expense reimbursement.

• Strong clinical assessment, communication, and teamwork skills.
