PATIENT INFORMATION
- Average Daily Volume: 18-20 patients per day
- Case Mix: Follow-up visits, new patient evaluations, treatment plans, medication management
FACILITY INFORMATION
- EMR System: Epic
- Specialty Backup Available: MD onsite 3 days per week
- Support Staff Available: Experienced Support Staff
JOB REQUIREMENTS
- Licenses: Active SC License, DHEC
- Other Qualifications: EPIC experience required
DUTIES & RESPONSIBILITIES
- Manage an average daily schedule of 18–20 patients.
- Conduct follow-up visits and evaluate new patients.
- Develop and update individualized treatment plans.
- Provide medication management in accordance with established clinical protocols and applicable regulations.
- Document patient assessments, treatment decisions, and care plans accurately and promptly.
