Job description We’re seeking a Nurse Practitioner to join our mission-driven FQHC, offering excellent benefits, a supportive team, and meaningful patient impact.

Job Responsibilities

Provide comprehensive medical care to a panel of adult patients on a continuing basis including but not limited to:
• Evaluation and management of acute and chronic medicalconditions
• Initial assessment and management of mental health conditions e.g. depression and anxiety disorders
• Ordering and analyzing necessary diagnostic tests using high value care principles and EBM to inform clinical decision-making
• Prescribing appropriate treatment and/or medication in an evidence-based manner and within the scope of licensure
• Advising patients on treatment recommendations, lifestyle modifications, and age-appropriate preventive screenings
• Referring patients for consultant services when necessary
• Engage in health promotion activities and recruitment efforts forth site
• Maintain accurate and detailed medical records about each patient, including observations during examinations
• Complete EMR documentation, billing, and patient-related paperwork in a timely manner
• Comfort working with diverse populations including recent immigrants to the United States
• Participate in night and weekend call shared among clinicians
• Collaborate with multidisciplinary team
• Attend and participate in clinic, provider, and all staff meetings
• Participate in pre-visit planning activities with the other members of the careteam
• Participate in center wide and clinic-specific quality improvement projects and peer review
• Participate in appropriate certification/maintenance of certification and continuing education activities
• Maintain updated license and DEA
• Work continually to assure deliverance of quality clinical care in a safe, efficient, competent, and effective manner
• Comply with all laws applicable to the practice of your specialty including HIPAA and mandated reporting
• Perform other duties as required or assigned

Qualifications

Master's Degree, specifically MSN (Master of Science in Nursing), Doctoral Degree, specifically DNP (Doctor of Nursing Practice) or equivalent
• Successful completion of an accredited nurse practitioner program
• Certification as a Family Nurse Practitioner or Adult Nurse Practitioner
• Current and valid NYS license
• Current DEA certificate
• Updated BLS and infection control certificate
• Excellent bedside manner
• AAHIVM certification and/or LGBT health certification is a plus
