Job description
We are seeking a dedicated and experienced Psychiatric Nurse Practitioner (Psych NP) to join our team in providing mental health services within homeless shelters and Federally Qualified Health Centers (FQHCs) in Brooklyn and the Bronx. The ideal candidate will have a strong background in trauma and substance abuse conditions and will work collaboratively with a multidisciplinary team to deliver comprehensive care to adults experiencing homelessness.
Function
Type of practice: Social Services, Mental Health Homeless Shelters, FQHCs co-located at both their shelters and others.
Practice site(s): 1 site in Brooklyn and/or the Bronx. There are needs in other locations in the other 3 boroughs of NY, with the largest need in Brooklyn and the Bronx.

Anticipated Dates of Assignment: ASAP.
Specialty / Clinical Setting: Mental Health Homeless Shelters.
Job Details
- Participates as a member of committees, including peer review, as requested by the CMO.
- Ensures chart completion is consistent with CFH P/P.
- Engages clients with appropriate concern for their behavioral health care needs.
- Coordinates medical care with PCP for chronic and acute non-behavioral health conditions.
- Advises and counsels clients about their physical and mental health; coordinates care with case management and health education as clinically indicated.
- Provides consultation and training to staff and volunteers where appropriate.
- Assists with the identification, counseling, and care coordination, with case management and health education, of mental health and substance abuse problems.
- Conducts intakes and medication management.
- The ideal candidate would have experience in trauma and substance abuse conditions.
Schedule Details
- Monday-Friday from 9a-5p or 8a-4p. Saturday from 9a-2p for volunteer and per diem for men and Sunday from 9a-2p for volunteer and per diem for women.
- Provider must work a minimum of 16 hrs a week. Can make 1st, 2nd or last 2 patients virtual to fit clinic hours while still allowing flexibility.

- Must be experienced providers with at least 1-2 years of experience.
