Job description
We are a growing, physician-owned medical group expanding in Evanston, IL seeking an experienced Nurse Practitioner to join our team in the acute setting. This role focuses on pre-rehab and rehab consults.
- Provide comprehensive medical care to Acute and ARF patients, including assessments, diagnosis, and treatment planning
- Manage patients with orthopedic rehab needs, post-stroke care, and chronic pain conditions
- Ensure accurate and timely documentation in EMR
Required Qualifications:
- Active Nurse Practitioner (NP) license in Illinois
- Active DEA license (or ability to obtain prior to start) - Required
- Minimum 1 year of clinical experience as an NP (ARF, geriatrics, internal medicine, or rehab experience strongly preferred)
- Experience managing adult/geriatric patient populations
- Strong clinical decision-making and time management skills
- Comfortable working independently in an ARF setting
Preferred Experience:
- Acute Rehab Facility (ARF) care experience
- Background in internal medicine, geriatrics, or physical medicine & rehabilitation (PM&R)
- Experience with orthopedic, stroke, or pain management patients
Schedule & Setting:
- Monday through Friday
- Full-Time
- No nights, no call, no weekends

Physical Requirements:
- Prolonged periods of standing, walking, and sitting while working with patients as well as working on a computer.
- Must be able to lift up to 50 pounds.
- May be exposed to bloodborne pathogens, physical hazards (e.g., needle pricks, etc.) and chemical hazards.
- Must wear the required PPE per company and/or facility requirements.
- Must comply with facility vaccination requirements.
