Partner with a physician-led PM&R group that provides consult-based care in Skilled Nursing Facilities across Texas, now expanding in Victoria. Known for combining high-touch patient care with high-quality documentation, we’re looking for exceptional NPs to join that mission.

The Role: You’ll collaborate closely with board-certified Physiatrists to ensure continuity of care and serve as a key presence within partner facilities. You’ll balance autonomy with physician guidance and play a vital role in restoring function and dignity for post-acute patients.

Job Responsibilities
· Ability to perform shoulder, knee, and trigger point injections without supervision
· Draft and contribute to high-quality documentation optimized for both clinical value and facility metrics
· Monitor patient progress and support functional recovery goals
· Collaborate with therapy and nursing teams to ensure comprehensive rehab care
Qualifications
· PM&R, Internal Medicine, Geriatrics, or Post-Acute experience preferred
· Team-oriented, communicative, and confident working in SNFs
· Passion for function-focused, patient-centered care
Working Hours
· No call, no nights: Focus on consultative care, not inpatient rounding or shift work
· Physician-led model: Supportive collaboration with Physiatrists who guide clinical quality, not volume quotas
· Full practice support: Admin, credentialing, billing, and scheduling handled for you
